It’s easy – with consent, complete a referral form for the person with dementia or care partner or both and send to your local Alzheimer Society. Although referring early/at time of diagnosis is optimal for patient benefit, you can refer at any point of disease progression.
First Link® partners include family physicians, geriatric specialists, nurses, social workers, occupational therapists, pharmacists, and social service workers who work in family practice, healthcare facilities and community agency settings.
